What Do I Need to Play?

STEP 1

This is the Athletics Physical Form. This must be completed by a Physician and brought to a WIHI Football Coach, or Athletic Trainer Bryan White. You MAY NOT participate in Athletics until this is completed. You need 1 Per school year. Click on the "PDF" Icon to Download and Print the Physical Form.

STEP 2

Another step that must be completed to participate in Athletics is to complete the Form-Releaf Athletics Registration at the following website. This is all student information and does not require a doctors visit.
